How can Nathan, our Community Link Worker help you?


The Community Link Project’s main aim is to bring together health, social care and voluntary sector professionals to work together and identify support for people who may feel lonely, isolated, fed up, unoccupied, looking for a social activity, hobbies or enjoyable form of exercise.

Nathan is available remotely to offer patients aged 18+, individualised, personal support and motivation.  You may require personalised solutions for social, emotional or practical needs that will empower you to make improvements to your health and wellbeing.
We all have issues in our lives which may affect our health in some way.  Nathan can help in the following ways;

  • Motivate and improve confidence in support of emotional wellbeing
  • Improve the management of your financial position.
  • Work on a relationship.
  • Provide access to a wide variety of services and activities provided by voluntary organisations and community groups throughout The East Riding of Yorkshire to support loneliness and isolation.
  • Know where to go for information relating to social care, (you may be a carer for a loved one and need guidance to manage your own wellbeing, or just generally feel better about yourself.
  • Support around organisations for benefits support, housing issues, loneliness.
  • Support and guidance for those patients looking for social activities or hobbies if you have become socially isolated.
  • General health & wellbeing guidance: stopping smoking, weight management, exercise and physical activity.

 
Community Link workers can spend up to an hour with you at each appointment and can meet as regularly as is necessary to help signpost and guide you to the right help, support and information.

Senaka also has access to Community Based workers that can support with home visits and they can also attend appointments as a buddy with a patient attending groups for the first time, as an example.

Dental Problems

We are unable to see or treat dental problems.  You would need to visit a dentist.  A full list of local dentists currently accepting NHS patients can be found on the NHS Choices website.

Smoking Cessation Support

If you require help with stopping smoking please contact the Health Trainers on 0800 9177752 or by visiting the Ready to Stop website.

Health Promotion

The practice is committed to encouraging patients to be responsible for their own health.  Our nurses are highly skilled in supporting patients to manage their own conditions and are always happy to see patients to give help and advice.

Physio Direct

If you feel you need to see a physiotherapist, you do not need to see a clinician.  You can self-refer to Physiotherapy. You can check your symptoms at www.chcpmsk.org.uk , and if you think you need to see a physiotherapist, fill in the online form or call 01377 208300.

National Screening Programme

All eligible patients registered with a GP in England are invited to participate in national screening programmes for cervical screening (smears), breast screening (mammograms), bowel cancer screening and retinal screening for diabetics.  You will be invited directly from the appropriate agency.  Further information regarding national screening programmes is available from the NHS website.

Private Services

The practice offers a full range of private services including HGV, PSV, taxi, employment and insurance medicals.  We also offer other non-NHS services such as completion of insurance reports and claim forms.  The current fees for private services are displayed at reception.  We follow the BMA scale of fees for services not provided by the NHS.  Appointments for private services are made outside of surgery time and are made by prior arrangement with a  GP and therefore are booked in advance.

Family Planning & Sexual Health

Routine and emergency contraception is available confidentially to all of our patients,regardless of age.  Dr Sally Anderson is trained to fit IUDs (Coils) and both Dr Anderson and Practice Nurse Linda Shipley are trained to fit the contraceptive implant.
 
Sexual health clinics are available locally, with walk in clinics available, alternatively you can refer yourself by calling 01482 247111 or visiting the Conifer House website.

Minor Surgery

Certain minor procedures and joint injections can be performed in the surgery by Dr Pedder-Smith.   The doctor or nurse would discuss this further with you if necessary,  Appointments for minor surgery are made outside of normal surgery time by the doctor’s PA and are made in arrangement with Dr Pedder-Smith. 

Travel Clinic

We are able to offer a fully comprehensive travel health advice service to our patients to ensure that you have had appropriate vaccinations and treatment prior to overseas travel.  Please complete a travel questionnaire form (available here) our nurses will research the most up to date information on your destination and you will be contacted to discuss your requirements.

Most travel vaccinations are not covered by the NHS and will be chargeable.
